We would like to welcome you to our office and give you a brief overview of what we so proudly provide during your initial exam. Your initial exam will include:

• A review of your medical and dental history
• X-rays as indicated
• Photos of your teeth (reviewed with you as both an educational tool and record keeping)
• An oral cancer screening
• Documenting existing restorations and missing teeth
• Identifying decay, cracks in teeth and failing restorations
• An occlusal examination (your bite and how it affects your teeth)
• A periodontic examination (to determine the health of your gums and supporting bone)
• TMJ examination (the health of your jaw joints)

Please note: a dental cleaning is NOT provided during the initial exam.

Upon completion of your initial exam, Dr. Mackey will review our findings with the aid of your dental x-rays, your photos, and any possible study models acquired.

Patient Forms

Upon reserving your first appointment with our office, you will be sent a welcome letter, a Health History Form, and a self addressed stamped envelope in the mail. It is appreciated if you could fill out your health history form and send it back to our office as to expedite your initial exam. If you are unable to send it to our office in time for your initial exam, please feel free to bring it with you on your first visit along with your photo ID and proof of insurance.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
